I certify and warrant that my crew and I understand the risks and dangers of offshore sailing and that the vessel and personnel aboard the vessel are fit for such passage.
I hereby release the Tucson Sailing Club and any other sponsoring group, its officers, agents, or committee members from any and all liability arising from the rally to the extent that such release does not discharge my insurance carrier under the provisions of the insurance policies on my yacht. I assume any risk of personal injury, death or property damage arising from my participation in the rally, failure or breakage of my yacht, or of any of its equipment or as a result of adverse weather conditions. I certify that I have apprised the Rally Committee of any performance altering modifications to my yacht.
A Copy of this Form MUST be  SIGNED by the Captain before participating
in the regatta.
